Op het formulier kun je vier uitgaveposten invullen, helaas kun je geen bijlage toevoegen en dien je de bonnen of facturen apart per e-mail naar de penningmeester te sturen.
Bij meer dan vier posten, moet je een nieuw formulier invullen.
Als je recent nog kosten gedeclareerd hebt en je bankrekeningnummer is niet gewijzigd, dan hoef je dat veld niet in te vullen.

The Dutch tunes are from the thirteen collections “Oude en Nieuwe Hollantse Boerenlietjes
en Contredansen” edited between 1700 and 1716 in Amsterdam.
The “Dutch” composer Julius Röntgen (1855 – 1932) arranged them in three collections used by Pat.
